Currently licensed in PA; services can only be rendered while you are physically within these States at the time of service.
Ages seen: Usually ages 16 and up (older than 12 is negotiable).
When submitting requests, please include the same details you would tell an in person physician. This may be online, but I still need to justify what I prescribe and know what I am treating, as well as have enough information to manage your dose. All refill requests for weight loss medication need to include your current weight; BP meds should include ballpark pressures, diabetics pls tell me your sugars, etc.
Without this information, I will have to ask more questions, and this can cause a delay in your prescription being sent.
Please turn on notifications for new messages from Push Health; it is best to download the app to your phone.
When you need a refill, best to BOTH put in a request via the push health menu / dashboard AND message me (this will ensure the fastest service). There have been instances of people putting a request in, but the request does NOT show up for me. Anyone who needs to talk to me can ask for a phone call.
I usually reply to messages quickly if I am at my computer, but if I am in the middle of office hours and seeing many patients, I can be delayed. If you do not have an active request, it is easy for your messages to get lost; if I do not reply within 14 hrs overnight or on weekends, or 6 hrs during office hrs, please message me again.
There is a $5 platform fee for changing prescriptions after they are sent.
There is no fee for switching pharmacies.
Many pharmacies will contact me for refills whether or not people actually want them. Therefore I do not accept refill requests from the pharmacy; all my patients must contact me for refills and let me know how they are doing.
If there is an issue with the prescription you must communicate it to me directly; do not assume that the pharmacy will successfully communicate with me.
I do not provide paper scripts to patients; all prescriptions are sent directly to the pharmacy.
Calling your insurance for any reason is not part of my service; if I have to call your insurance, additional admin fees may apply.
CVS has a higher rate of not getting my fax the first time. If after 1 hr the pharmacy still does not have it, please ask me to re-send the fax, or consider changing the pharmacy.
Re-faxing of a prescription due to the pharmacy not receiving it will only be done up to 2 wks from the date of the original prescription; after that it will be considered a new service.
In order to keep my fees low, calling a pharmacy to find out what an issue is is generally not part of my service. If there is an issue with your prescription, I need you to relay it to me with as much details as possible; this is better than relying on the pharmacy to communicate it to me, as any faxes they send me get lost in spam.
$99 for most consultations, which includes for acute reasons, medication refills, and/or follow ups.
Most issues can be handled via messaging, however if you would prefer a phone or video call, please state so in the request or message thread. Otherwise, this will be up to medical necessity as I deem appropriate. Please do keep in mind that video or phone calls would need to be worked into my current patient schedule so may delay the delivery of your care.
Lab ordering fees generally vary from $100 to $300 depending on how many labs are ordered; the fee covers interpretation as well. The labs ordered via this platform are not covered by insurance, so if you have a PCP who can order them with your insurance, I'd recommend requesting they do so before requesting here.
Phone appointments are "on the fly" (you message me, then I call back when I can); I can try to do time windows.
Steps for an “on the fly” appt:
1. Make sure I have the data I need, such as labs etc.
2. Message me on Push health: example “I can take your call from now till (time) pm”, or “I can take your call from 1 pm to 3 pm”. Make sure Push Health has your correct phone number.
3. After I see your message, I will call when I can. If I cannot make the call, we try again another time or another day.
As mentioned previously, communication via messaging is preferred in order to facilitate timely delivery of your medical care.
Controlled substances (phentermine, xanax, ambien, adderall, testosterone, fioricet, etc) absolutely CANNOT be prescribed. The only medication I can prescribe for ADHD is Strattera, Qelbree, Intuniv/Guanfacine, or Wellbutrin.
Medicine is legally practiced at the current physical location of the patient. At the time service is delivered, you need to physically be in a State I am licensed in (PA).
If you are traveling out of State and need help, it is best to create a second Push Health account under a different email with your out of state address.
Filling out any insurance form for medication is not part of my consultation service. This includes completing prior authorizations. All other forms and letters (only on a case by case basis) are subject to fees which are typically an additional consultation fee of $99.
The Prior Auth process is often not quick, and can take 2 to 4 wks.
Emergency/Urgent services are not provided; if you have a medical emergency, please go to the nearest Emergency Room or Urgent Care.
I do not accept insurance, and do not provide forms for insurance reimbursement.
